Great Ormond Street Hospital (GOSH) partnered with Efficio to identify and deliver sustainable savings across its non-pay expenditure, supporting the Trust’s wider financial objectives. The ambition was not simply to identify opportunities on paper, but to translate them into real savings and strengthen GOSH’s ability to sustain improvements over the longer term.
The challenge: Identifying practical, deliverable opportunities
GOSH was looking to identify opportunities to optimise non-pay expenditure across a diverse range of clinical and non-clinical categories. The complexity of the spend, alongside data and contractual considerations, meant that identifying opportunities required more than a traditional spend review. Solutions needed to be evidence-based, operationally practical and developed with the clinical and operational teams responsible for implementing them.
The approach: From assessment through to delivery
Efficio worked alongside GOSH from initial opportunity assessment through to implementation:
- Used spend analytics, benchmarking, contract and supplier reviews, and stakeholder engagement to identify and prioritise opportunities across clinical and non-clinical categories.
- Validated opportunities with stakeholders to ensure they were commercially worthwhile and operationally achievable.
- Deployed a broad range of procurement levers, including competitive sourcing, supplier negotiation, demand and specification optimisation, supplier consolidation, and alternative delivery models.
Efficio and GOSH worked closely together throughout the programme, combining Efficio’s deep public sector and healthcare expertise and experience with GOSH’s knowledge of its services, suppliers, and operational requirements. GOSH retained clear ownership of decisions, with the Efficio team offering hands-on support to move initiatives from opportunity through validation to implementation. Structured governance and regular performance tracking helped maintain momentum, also keeping progress and savings visible to the Trust.

Over the programme, the joint GOSH and Efficio team delivered more than 26 initiatives across clinical and non-clinical spend, generating 13% cost savings for the Trust through a blend of sourcing, negotiation, demand management, and alternative delivery models.
Beyond the financial impact, the programme strengthened savings governance and tracking, sourcing and contract management disciplines, and clinical and operational involvement in Procurement-led improvement. And, importantly, GOSH was left with a repeatable approach to identifying, validating, and delivering future savings opportunities, supporting continued improvement beyond the programme.
